Your First Visit

A consultation with Dr. Nishimoto or Dr. Wessberg and one of our specially training staff during your first visit to our office is essential for us to provide you with optimum health care. This visit will include preparation of your medical/dental record, history and examination related to your problem, a review of your treatment alternatives with respect to risks and benefits, and an itemized state of the professional fees.

Please Bring The Following Information With You To Your First Visit:

-List of all medications that you are currently taking
-Medical History
-Medical and/or dental insurance information (if applicable)
-Photo ID

X-rays from your referring dentist may have been sent to our office ahead of your appointment. Contact your referring provider if you are unsure if you need to bring your referral form.
